Patient outcomes and the fulfillment of care goals is one of the major ways that healthcare success is measured. Measuring patient outcomes results in the generation of data that can be used to improve results. Nursing informatics can have a significant part in this process and can help to improve outcomes by improving processes, identifying at-risk patients, and enhancing efficiency.
The Impact of Nursing Informatics on Patient Outcomes and Patient Care
Among the key goals and objectives of patient care is offering high-quality care, ensuring that patients are safe, and having a better experience. The implication is that various stakeholders in the continuum of care have explored various strategies to achieve the goals. Technology and technological applications have formed the core of such efforts; among them is nursing informatics which has revolutionized patient care to influence outcomes and care efficiencies (McGonigle & Mastrian, 2021). The purpose of this assignment is to write a proposal of a nursing informatics project to my healthcare organization leadership to improve patient outcomes and patient care efficiencies. The paper will describe the relevant stakeholders likely to be affected by the project and possible patient outcomes. In addition, this proposal will also address the technologies that can be utilized to implement the project.
The Proposed Patient Outcome Improvement Project
As earlier indicated, nursing informatics has been used to great effect to influence both patient outcomes and patient care efficiency. The proposed informatics project will ensure that patients adhere to their clinic follow-up schedules; therefore, it is known as Clinic Visit Adherence Reminder System. This system is nurse-focused and will be applied by nurses in managing the clinic follow-up for patients diagnosed with various chronic conditions such as obesity, diabetes, and hypertension. Adherence to follow-up is key to improving a patient’s condition (Jäwert et al., 2021). When a patient comes to the clinic for the first time, the nurses carry out routine checks and screening for the mentions condition. Upon the confirmation of one or more of the conditions, the patient details are fed into the clinic visit reminder systems with unique identifiers specific to patients.
The nurses will also infeed the diagnostic results and write recommendations for a physician examination and a clinic booking for the individual. Such details are key in alerting the doctor when performing further checks on the patient. In case the physician makes a decision that the patient should start attending a clinic session, the system chooses a booking time and date and states whether the patient needs more urgent attention. In subsequent clinic visits, the nurses get alerts once the patient’s unique identifying number is fed into the system, indicating whether the patient has been honoring the previous clinic visit dates. The implication is that the nurse can easily track the patient’s data on clinic visits with a click of the mouse.
Depending on how bad an individual is fairing in terms of honoring the clinical visits, the nurses, in collaboration with other healthcare professionals, can formulate plans to ensure that the patients come back to the clinic as booked. Indeed, it is well known that patients living with such chronic conditions usually have worse outcomes if they do not adhere to clinic visit schedules and medication regimens (Mariam et al., 2017). Clinic visit adherence reminder system will therefore be important in helping and reminding the nurses of the patients who are not adhering to scheduled follow-ups and ensuring that there is a collaboration with other healthcare providers to ensure correct scheduling of clinic appointments and formulation of plans to ensure that the patients adhere
